Hazing is a term that many people are familiar with, but not everyone thinks of it in the same way. Hazing can be defined as â€Å"any act that endangers the mental or physical health or safety of a student, or that destroys or removes public or private property, for the purpose of initiation, admission into, affiliation with, or as a condition for the continued membership in a group or organization.† The issue of hazing can be controversial at times because some feel that it is nothing more than a way for a group to bond, but others fight against all kinds of hazing because of their belief that hazing is a serious threat to those who have to endure it, especially the youths of the world. Hazing is most often associated with fraternities, but it is an issue in many different organizations, including sororities, the military, athletic teams, student clubs (such as marching bands), and in high schools. Those who have never been involved in hazing may wonder why the victim would agree to the often humiliating or dangerous initiation rituals. The most common answer to this question is that they participated because they believed it would give them a sense of belonging, despite the fact that these so-called traditions may harm the participants. Most anti-hazing laws make it clear that it does not matter if a person consented to be subject to hazing because of the pressure they are under at the time and their lack of knowledge about what the process would involve. One of the most common forms of hazing, especially within fraternities and sororities, is the forced consumption of alcohol, often in lethal doses. One such incident involved Chuck Stevens, who was taken from his dorm in New York State, locked in the trunk of a car, and forced to drink a mix of bourbon, wine, and beer. Chuck died as a result of being hazed, and his mother, Eileen, has since founded the Committee to Halt Useless College Killings, or C.H.U.C.K., in memory of her young son. This organization promotes awareness and educational programs, tries to find alternatives to hazing, and works towards stricter anti-hazing laws. People’s perception of exactly what hazing is often varies form person to person, which makes it hard to separate acceptable traditions from unacceptable traditions. Even people who have witnessed hazing rituals or been a part of a hazing ritual are reluctant to label it â€Å"hazing† because of the stigma associated with the word. Victims of hazing will often not report the incident because they are unsure if what they suffered is classified as hazing and they are afraid of no longer being a part of the group they had worked so hard to join. A study conducted at Alfred University explored the various reasons why anyone would want to participate in a hazing ritual. Nearly half of those surveyed answered because they thought it was fun and exciting, but the majority of those students were only subjected to humiliating hazing. Other reasons include they were scared to refuse, they felt it brought the group closer, they wanted to get out some aggression through fighting, and they were immature when they agreed to the hazing. The study also discovered that many students did not know whether hazing was illegal in their state, and that if there was a law, it made no significant difference in the level of hazing behaviour. Hazing is something that occurs much more than we are aware because most incidents go unreported. Those students who did report hazing also reported both positive and negative consequences of the actions committed by them and by others. A common occurrence among those hazed was depression, which often included anger, and if the hazing was physical, the victim may suffer internal bruising and pain. Not all hazing has such serious penalties, though, and one positive aspect that was reported by students is that they felt they had gained a valuable life experience and had been able to mature. The outcome of hazing is different for each person who goes through it and it all depends on what type of hazing they endure and for what purpose they felt it was worth agreeing to. Aside from hazing taking place in the fraternities and sororities of universities, it is also a dominant problem in schools athletic teams. † Nearly 80 percent of student athletes reported being subjected to one or  more hazing behaviours, ranging from shaving their heads to being tied up and paddled, as part of their team initiations.† Amazingly, only twelve percent of these students were willing to call what they had gone through hazing. Many rookies, especially males, are also forced to humiliate themselves and others through some kind of sexual abuse or nudity, such as walking around naked while holding another mans genitals. Members of the University of Vermont’s hockey team in reportedly carried out this shocking behaviour in October of 1999. Former UVM hockey player Corey LaTulippe, who filed a civil lawsuit against the university, has told the media that he was repeatedly subjected to hazing by other members of the team, and that school officials did nothing to end it. Women’s athletic teams also face hazing rituals, but they are more likely to embarrass their rookies, where men are more likely to be involved in extreme hazing, such as beatings or theft. In addition to hazing being a major component of universities and high schools, it is also an issue that the military must deal with constantly. Military hazing â€Å"can be used to describe anything from a good-natured punch on the stripes when someone is promoted, to Navy chiefs who make a new chief wear a dress, to boot camp activities when superiors or peers try to transform a balky recruit into a trustworthy team player.† Hazing is an issue for branches of the military all over the world, including Canada, the United States, and Russia. Hazing in the Russian armed forces can be particularly harsh, considering that many first-year soldiers die at the hands of their â€Å"grandfathers,† or their superiors. In the United States, the public knew nothing about military hazing until 1956, even though it began over a century ago. Just a few short years ago, there was a scandal that rocked the military when videotapes of Marines participating in blood pinnings were released to the media. This initiation ritual involved Marines ramming gold-wing pins into the chests of fellow Marines, which sickened all those who viewed the graphic footage. Despite the many dangers that are associated with hazing, experts warn against eliminating it altogether. Sociologist Jamie Bryshun has said that â€Å"the traditions often run too deep, and nothing any coach or university administrator says or does can end them.† There is fear that if all hazing  is completely forbidden, then the organizations will just take it underground, where it may become even more dangerous. Different groups have tried to come up with alternatives to hazing, such as planning special events as a way for the members to bond with each other. The penalties for participating in hazing are on the rise. Hazing is now illegal in 41 states in the U.S.A., and many are working towards increasing that number. Under Texas law, hazing has been committed if a person engages in hazing, solicits, encourages, directs, aids, or attempts to aid another in engaging in hazing, intentionally, knowingly, or recklessly permits hazing to occur, or has firsthand knowledge that a hazing incident is being planned or has occurred and does not report their knowledge to the proper authorities. Even if the person has agreed to be a part of the hazing activity, it is still classified as hazing and charges could still be filed. Some criminal penalties include: a fine up to $1000 and/or up to 180 days in jail for failing to report hazing, a fine of $1000-$5000 and/or 180 days to one year in jail for being involved in hazing that resulted in serious bodily injury, and a fine of $5000-$10000 and/or one to two years in jail for participating in hazing that resulted in the death of someone. Despite the increase in awareness about the subject of hazing and the tougher anti-hazing laws, it is still a major problem for organizations all over the world. Even though many have died and countless others have been hospitalized, the trend of dangerous hazing has continued to grow, and it will not end until groups such as universities and the military receive a major wake up call. It is up to the leaders of the organizations to ensure that all its members are made to feel safe and no longer pressured to participate in activities that they know could endanger their lives.

Treatment for Digoxin Overdose B. Trimble Digoxin Overdose Digoxin is derived from the leaves of a digitalis plant (foxglove). Some plants have chemicals that can cause symptoms similar to digoxin if eaten, such as lily of the valley and oleander. Digoxin is a substrate of P-glycoprotein. Drugs that induce or inhibit P-glycoprotein in the intestines or the kidneys have the potential to alter digoxin pharmacokinetics (Katzung, Mastes, Trevor, 2012). Digoxin increases the strength of heart contractions by inhibiting the activity of the enzyme ATPase. ATPase controls the movement of calcium, sodium, and potassium into the heart muscle. ATPase increases the amount of calcium in heart muscle, which increases the force of contractions. Digoxin slows the electrical conduction between the atrium and ventricles of the heart and slows ventricular contractions. Digoxin is eliminated through the kidneys and should be reduced in dosage in patients with kidney dysfunction (Katzung, Mastes, Trevor, 2012). Medications such as verapamil, quinidine, Amiodarone, indomethacin, spironolactone, Alprazolam and itraconazole can increase drug levels and the risk of toxicity of digoxin. Furosemide and other diuretics that reduce blood potassium or magnesium levels may predispose patients to drug induced abnormal heart rhythms. Saquinvard and ritonavir increase the amount of digoxin in the body and may cause toxicity (PubMed, 2013). Assessment of the severity of toxicity and etiology (accidental, unintentional, or deliberate overdose) altered drug metabolism due to decreased renal function or interaction with other drugs is necessary. Consideration of factors that influence treatment include age, medical history, chronicity of digoxin intoxication, severity of heart disease, and/or renal insufficiency and ECG changes (Katzung, Mastes, Trevor, 2012). Symptoms of digoxin toxicity include anorexia, nausea, vomiting, diarrhea, visual changes, cardiac arrhythmias (1st degree, 2nd degree â€Å"Wenckebach†, or 3rd degree heart block), atrial tachycardia with AV block, AV dissociation, accelerated junctional, unifocal or multifocal premature ventricular contractions, ventricular tachycardia, and ventricular fibrillation (Patel, 2011). Toxicity is usually associated with levels greater than 2 mg/ml. Low body weight, advanced age, impaired renal function, hyperkalemia, hyper-calcemia, or hypo-magnesium may cause d igoxin toxicity. Other symptoms may include decreased consciousness, decreased urine output, difficulty breathing, and overall swelling (Brunton, Chabner, Knollman, 2011). Treatment will consist of emergency protocol if outside the hospital; this includes calling emergency medical services and CPR. Once the patient is in medical care, the treatment will depend upon the severity of symptoms and levels of digoxin in the body. Laboratory testing will include serum electrolytes, digoxin levels, and thyroid function tests. The patient will be placed on continuous cardiac monitoring with a 12 lead ECG obtained (Brunton, Chabner, Knollman, 2011). The primary focus is to correct electrolyte levels; if hypokalemic administer potassium to reach a level of 4.0 to 5.5 mmol/L. Activated charcoal will be administered either orally or per nasogastric tube in order to bind undigested digoxin. If bradycardic and symptomatic, atropine may be given intravenously. Peak cardiac effects of digoxin occur 3 to 6 hours after ingestion. Gastrointestinal symptoms precede cardiac manifestation. Neurological symptoms like fatigue and malaise are common. Visual disturbances occur with aberration in color vision, mostly yellow-green. Activated charcoal binds to the digoxin and prevents recirculation to the enterohepatic circulation. Cholestyramine may be used for chronic toxicity in patients with renal insufficiency. Continuous hemodynamic monitoring includes the ECG and 12 lead EKG (Brunton, Chabner, Knollman, 2011). Prompt measurement of electrolyte levels (potassium, calcium, digoxin, BUN, creatinine, and CMP). Sodium bicarbonate may be administered to correct metabolic acidosis along with glucose and insulin to enhance potassium uptake by the cells (Brunton, Chabner, Knollman, 2011). Magnesium may serve as a temporary antiarrthymic until digifab is available. Hypomagnesium increases myocardial digoxin uptake and decreases cellular sodium/potassium ATPase activity. Digibind (digifab or digoxin immune Fab) is an immunoglobulin fragment that binds with digoxin. In acute intentional overdose digibind (40 mg reconstituted with 4 ml sterile water) is administ ered 4 to 6 vials as a loading dose over 30 minutes as an emergent IV bolus. The bolus is followed by 0.5 mg/minute for 8 hours and then 0.1 mg/ minute for 6 hours (Patel, 2011). For patients with chronic toxicity that are dependent on digoxin, the initial dose is twice the bolus. This avoids complete reversal of clinical effects of digoxin. Response is typically within 20 to 30 minutes after infusion, elimination half- life is around 16 hours. Digoxin levels are unreliable for one to two weeks after therapy. Complications in long-term digoxin users, who receive digibind treatment administration are that it may precipitate worsening of heart failure as reversing the beneficial inotropic agent of digoxin causes hypokalemia and atrial arrhythmias with rapid ventricular response (Katzung, Mastes, Trevor, 2012). Hypokalemia has occurred in patients treated with standard therapy as well as with Fab fragments. Clinically adverse phenomena have occurs in patients with immunotherapy. Other untoward effects of Fab include anaphylaxis and serum sickness, this is because it is a sheep protein, but this is uncommon. Recrudescence of digoxin toxicity is possible within 7 to 14 days because Fab is eliminated more rapidly than digoxin released from tissue binary sites. Plasmapheresis may be performed or the agent reinstituted in such cases (Patel, 2011). If hemodynamically stable, bradycardia and supraventricular arrhythmias may be treated with observation and supportive measures. Ensuring hydration to optimize renal clearance, administering gastrointestinal binding agents may be used. For patients with rate related ischemia or neurological unstable digiFab is the treatment of choice (PubMed, 2013). In unstable premature ventricular contractions, lidocaine may be effective. In ventricular tachycardia the best response is to digiFab, but phenytoin and lidocaine are useful if Fab is ineffective or unavailable (Brunton, Chabner, Knollman, 2011). They depress the enhanced ventricular automaticity without significant slowing of AV conduction. Phenytoin may reverse digoxin induced prolongation of AV nodal conduction. Phenytoin has been shown to dissociate the inotropic and dysrhythmia actions of digoxin, suppressing digoxin tachycardia without diminishing the contractile affect and can terminate SVT induced by digoxin. Doses for lidocaine are 100 mg bolus with an infusion of 1 to 4 mg/minute. Phenytoin dosage is 100 mg every 5 to 10 minutes up to a loading dose of 15 mg/kg. Magnesium sulfate dosage is 2 gram over 5 minutes followed by an infusion of 1 to 2 g/hour, with magnesium levels drawn every one to two hours. Atropine may be given for bradycardia to improve sinus and AV node conduction by inhibiting vagal activity (Brunton, Chabner, Knollman, 2011). Phenytoin may reverse digoxin induced prolongation of the action potential in myocardial cells and may suspend tachycardia, prolongs effective refractory period, and depresses spontaneous depolarization in ventricular tissue. Lidocaine is a class IB antiarrthymic that increases the electrical stimulation threshold of the ventricles, suppressing the automaticity of conduction through the tissue. It combines with sodium channels and inhibits recovery after repolarization, resulting in decreased myocardial excitability and conduction velocity (Brunton, Chabner, Knol lman, 2011). Magnesium sulfate possesses properties that slow the rate of sinoatrial node impulse formation and prolong conduction times (Brunton, Chabner, Knollman, 2011). Prevention of unintentional overdose (accidental overdose, interaction with other medications, or the altered metabolism due to renal insufficiency) is mostly through patient education. Instructing the patient in the correct dosage of the medication; that blood tests will be necessary to ensure appropriate dosage; suggesting daily recording of heart rate and blood pressure. During this time period, there were two types of theatrical performances that were available for the people’s viewing, comedies or tragedies. These two genres were never really intertwined until the time of William Shakespeare. His play, Romeo and Juliet, is an example of both a comedy and a tragedy. It starts off as a comedy with Romeo weeping like a baby because of his love Rosaline, who did not love him back and ends as a tragedy when Romeo and Juliet, a pair of star crossed lovers, commit suicide because the lost of each other. It was also during Shakespeare’s time that writer were finally acknowledged by the people. Befor e this time, writers were not considered upper classman. Another group of people that began to rise into a higher social class were the actors. Actresses were not present back then because women were not allowed on stage. It was considered unladylike to have a female actor. Men played all the parts. Theater owners were dependent on actors to make them a profit. Rehearsals for the plays were fairly short, only lasting for about a week. The performances themselves would only show for three to four days. Actors were expected to memorize hundreds of lines at a time. While one play could be performing, actors would be practicing lines for their next show. Play writers also began to make roles for the actors in the theatrical pieces. The theaters that actors performed in were roofless so that the sun could be used as lighting. Theatrical shows were held in the afternoon because it provided the best amount of light for the show. When the people gathered into the theater, the different clas ses of people were separated by where they could afford to sit and watch the show. The lower classmen were situated on the bare earth where it was dirty and smelly because it was never cleaned. The owners’ of the theaters found it less expensive if they did not keep high maintenance of their establishments. Higher classmen sat under a roof and for a penny more, they could buy cushions for their seats. Reforma Tributaria y Modernizacià ³n de la Administracià ³n Tributaria En mayo de 1992 se promulgà ³ un nuevo cà ³digo tributario que modificà ³ sustancialmente el sistema tributario anterior. Entre los objetivos que se trazà ³ esa reforma tributaria estuvieron: 1) reducir distorsiones que generaban las elevadas tasas de impuestos sobre los ingresos; 2) disminuir el sesgo que generaba el anterior sistema contra el ahorro; 3) elevar el grado de elasticidad del sistema impositivo a los cambios en los precios; 4) aumentar el peso especà ­fico de impuestos de amplia base y baja tasa, a fin de reducir la distorsià ³n que generaban los impuestos en el sistema de asignacià ³n de recursos; 5) reducir la evasià ³n fiscal; y 6) aumentar las recaudaciones y la presià ³n tributaria.
